Important things to note
Once you have submitted the form above, you’ll need to advise your friend to:
1. If signing up with a MATE sales agent over the phone:
Tell the agent your existing account name and MATE code/account number
2. If signing up online:
Complete the “how did you hear about us” section of the online sign-up form with your full account name and MATE code/account number
For both online and phone sales, the referral details MUST be left at the time of the order – these can not be added at a later date once the order has been submitted.
